What Is Melasma?

Melasma occurs when the skin produces excess melanin, the pigment that gives skin its color. While the exact cause is unknown, certain factors are known to trigger or worsen the condition, including:

  • Sun exposure: UV light stimulates melanocytes (pigment-producing cells), increasing melanin production and darkening the skin.
  • Hormonal changes: Pregnancy, birth control pills, and hormone replacement therapy are often associated with melasma due to the fluctuation in hormones.
  • Genetics: A family history of melasma can increase the likelihood of developing it.
  • Skin type: People with darker skin tones are more prone to melasma.

Signs and Symptoms of Melasma

Melasma is characterized by symmetrical brown or gray-brown patches, which often appear on the face. Common areas affected include:

  • Cheeks
  • Forehead
  • Upper lip
  • Nose
  • Chin

The patches tend to be darker and more pronounced in areas that get more sun exposure, and they can vary in size and shape.


Diagnosis of Melasma

At Desert Sage Dermatology, Megan Ahmed, NP-C can easily diagnose melasma through a thorough clinical evaluation. In some cases, a Wood's lamp examination (a special ultraviolet light) may be used to examine the depth of the pigmentation in the skin. This helps determine the most effective treatment options.

Melasma Treatment Options

While melasma cannot always be completely eliminated, there are several treatment options available to manage and reduce the appearance of the dark patches:


1. Topical Treatments:

  • Hydroquinone: A lightening agent that helps reduce pigmentation.
  • Tretinoin: An effective retinoid that accelerates skin cell turnover and reduces discoloration.
  • Corticosteroids: To reduce inflammation and enhance the effect of other treatments.
  • Azelaic Acid & Vitamin C: These are gentle alternatives for treating pigmentation while also promoting skin health.

2. Laser Treatments:

  • Fractional CO2 lasers or picosecond lasers can be used to target pigment in the skin without damaging the surrounding tissue. These treatments break up the melanin and reduce the appearance of dark spots.

3. Chemical Peels:

  • Special chemical solutions that exfoliate the skin and help fade pigmentation. These peels can be customized to treat melasma based on skin type and severity.

4. Sun Protection:

  • The most crucial step in managing melasma is proper sun protection. UV exposure can make melasma worse, so a daily broad-spectrum sunscreen with SPF 30 or higher is a must. We also recommend wearing hats and avoiding peak sunlight hours.

5. Oral Medications:

  • In some cases, oral treatments like tranexamic acid may be prescribed to help lighten melasma, especially in more persistent cases.
